I always structure my classes around real-life themes (how to introduce yourself, how to make an appointment, how to shop, how to go to the doctor, talk at work, etc). Then, we will identify the student's biggest worries regarding the English language — where they are confused, or tend to make mistakes. This will form the workshop part of the lesson, focused on overcoming tricky grammatical mistakes and revising more complex linguistic and phonetic errors.
